Dear Parent/Guardian,
I am very pleased that ________________________ is in Health Education class. With your support and encouragement, your child will be part of a nurturing and exciting learning environment.
Since much of life's success depends on wise decision-making, I have developed a classroom discipline plan that affords every student the opportunity to learn and succeed in a positive environment. Below is an outline of our class rules and consequences.
Rules: 1. Be on time with three ring binder with loose leaf paper, pen/pencil, index cards.
2. Follow direction the first time.
3. Raise your hand and wait to be called on before speaking.
4. Keep hand, feet, and other objects to yourself.
5. Stay seated until dismissed.
To encourage students to follow these rules, I will make every effort to recognize and support appropriate behavior. However, if a student chooses to break a rule, the following steps will be taken:
First time a student breaks a rule: Reminder
Second time: 1 minute after class with teacher
Third time: 10 minutes working away from group or after school detention
Fourth time: Call parents/Guardian
Fifth time: Send to office
It is my goal that your child is successful and enjoys a good learning experience in my classroom. Please read this classroom discipline plan with your child, then sign and return the form below.
Sincerely,
Mrs. Bonczek
